Job summary

DiscoverU Health is building a concierge-style medical practice in the DMV area, seeking Licensed Clinical Social Workers and Registered Nurses to join as Concierge Health Coaches in a per diem role. You will develop holistic care plans, coordinate referrals and follow-up services, help members navigate care, and document progress while collaborating with a small, mission-driven clinical team.

Qualifications

We’re building a team of care plan specialists who love partnering with members to design personalized care pathways that help them live healthier, happier lives.

Our Concierge Health Coaches will help bridge medical care with everyday wellness by coordinating services, resources, and follow-up care that support each member’s unique goals.

As we launch this fall, we’re looking for Licensed Clinical Social Workers and Registered Nurses to join us in this meaningful work. This is a per diem role with 6 or more hours per week that may grow into part-time or full-time as our practice expands.

Role Overview

As a Concierge Health Coach, you’ll work closely with members to build holistic, actionable care plans — ensuring they have clear next steps, easy access to resources, and personalized support to stay on track.

Key Responsibilities

• Complete intake assessments and collaborate with members to develop individualized care plans.
• Coordinate referrals, follow-up appointments, and needed services.
• Act as a point of contact for members, helping them navigate their care.
• Monitor progress and update care plans as needed.
• Maintain clear, timely documentation.
• Collaborate with the clinical team to ensure whole-person support.
• Share insights to help strengthen the overall member experience.

Requirements

• Active license in D.C., Maryland, or Virginia as either:
  • Licensed Clinical Social Worker (LCSW)
  • Registered Nurse (RN)
• Experience in care coordination, case management, or concierge-style care preferred.
• Credentialed with major insurance providers strongly preferred.
• Strong commitment to preventive care and personalized member support.
• Excellent communication and organizational skills.
• Ability to work independently and collaboratively with a small, mission-driven team.
