We are currently hiring for a Concierge Care Manager to join BlueCross BlueShield of South Carolina. In this role, you will provide care management services to over 2 million members in South Carolina and nationally. Your responsibilities will include administering complex, high-touch concierge case management, identifying risks, and facilitating healthcare service coordination to improve quality of life and care satisfaction. The position is remote, working from home, primarily via phone and messaging system, serving members with chronic or high-risk conditions.
