Job description

Join to apply for the Care Manager, Telephonic Nurse-Part Time After Hours & Weekends role at CenterWell Home Health.

This position is part-time, 20 hours/week, scheduled on weekdays after business hours and weekends.

Role Overview

The Clinical Care Coordinator ensures optimal continuity of care for patients transitioning into and out of our services. They are responsible for understanding post-acute care levels, communicating with the clinical team, and facilitating timely patient follow-up. This is a work-from-home telephonic nurse position supervised by the Manager of Care Coordination.

Essential Functions
  • Support discharged patients or those needing post-acute services, navigating healthcare options, billing, and resources.
  • Use various tools to provide patient education and handle customer issues.
  • Determine clinical care needs based on discussions and records, matching patients to appropriate services.
  • Maintain awareness of department goals, ensure patient satisfaction, and assist in the admissions process.
  • Focus on placing patients in appropriate care settings timely.
  • Participate in mandatory training and adhere to company policies.
  • Provide education on Home Health, Hospice, and Palliative Care Services.
  • Assist with clinical eligibility reviews and participate in projects as assigned.
Minimum Qualifications
  • Associates Degree required; BSN preferred.
  • RN license in a compact state required; additional states may be needed.
  • At least 3 years of post-acute care experience; home care or hospice preferred.
  • Current CPR certification.
  • Strong knowledge of home health, hospice, and palliative care.
  • Excellent communication, analytical, and problem-solving skills.
  • Fluent in English, both written and spoken.
Additional Information

Home internet must meet minimum speed requirements; satellite or cellular connections need approval. Employees in certain states will receive internet expense support. A dedicated, interruption-free workspace is required to protect patient information.

Benefits

Compensation

Base pay range: $71,100 - $97,800 annually, depending on location and experience. Full-time hours are 40 per week.
