Job summary

Duke Regional Hospital in Durham, NC, is seeking a Case Manager to coordinate discharge planning and care transitions across the continuum. The role partners with interdisciplinary teams to ensure timely assessments, efficient referrals, and optimal patient outcomes.

Requires BSN or MSW, minimum 3 years of relevant experience, and Case Management Certification within 3 years; NC RN license for BSN holders. Relocation assistance may be available.

Qualifications

• BSN or MSW required.
• Minimum of 3 years of relevant experience.
• Case Management Certification required within 3 years of hire.
• For BSN-prepared candidates: Must hold a current RN license in North Carolina.

Responsibilities

• Assess assigned patients for case management needs and discharge planning.
• Develop, implement, and evaluate individualized discharge plans.
• Coordinate services such as Home Health, DME, transportation, SNF placement, and post-acute referrals.
• Monitor daily census to ensure evaluations are performed.
