The Select Community Nurse Care Management (NCM) Program is seeking a registered nurse (RN) to manage members with intellectual and developmental disabilities in Putnam County. This role offers an opportunity to positively impact the lives of individuals by assisting with their health needs through in-home and telephone contact, managing complex cases with multiple needs.

Note: Besides clinical and healthcare experience, candidates must have 2 years’ experience in IDD.

Job Responsibilities

• Supporting utilization management functions for complex, non-routine cases.
• Serving as a liaison between members, providers, and internal/external customers to coordinate healthcare delivery and benefits.
• Overseeing highly complex cases to ensure effective interventions and optimal utilization of benefits.
• Performing core case management activities: assessment, planning, implementation, coordination, monitoring, outcomes, and evaluation.
• Communicating effectively via digital channels and providing technical support.
• Note: This position requires an 18-month commitment before applying for other internal roles, effective from 7/22/13.
• Additional immunizations or medical tests may be required.
• Requires digital literacy assessment.

Job Qualifications

License

• Active Registered Nurse (RN) license in Tennessee or a license in the resident state if participating in the Nurse Licensure Compact Law.

Experience

• 3 years of clinical experience.
• 5 years in the healthcare industry.
• For Select Community & Katie Beckett: 2 years in IDD is required for Select Community.

Skills & Certifications

• Certified Case Manager (CCM) credential or must obtain within 2 years of hire.
• For Select Community & Katie Beckett: Certification in Developmental Disabilities Nursing (CDDN) is required at hire or within 3 years.
• Excellent communication skills (oral and written).
• Basic PC skills (Microsoft Office, Email).
