The Nurse Case Manager, RN, manages patient cases for ongoing engagement, discharge planning, transition of care, and outpatient management. Responsibilities include screening, monitoring, developing care and discharge plans, conducting assessments (telephonic and face-to-face), collaborating with an interdisciplinary care team (IDT), acting as patient advocate, and participating in care plan reviews. The role involves guiding patients and families through the healthcare system for transition and ongoing care, working under supervision of an RN Manager or MD.

This position offers the flexibility to work remotely from anywhere in the U.S., with hours from 8 am to 5 pm in your time zone.

Primary Responsibilities:
  • Conduct transition of care calls and assessments to ensure discharged patients receive necessary services.
  • Serve as a clinical liaison for discharge planning and care needs.
  • Make referrals to community resources.
  • Engage with patients and caregivers to establish and assess care plans.
  • Provide education on self-management and disease management.
  • Develop interventions based on evidence-based practices, considering member barriers.
  • Use motivational interviewing and review health history to understand patient needs.
  • Conduct initial assessments for complex cases within designated timeframes.
  • Manage treatment plans and collaborate with healthcare providers.
  • Participate in IDT conferences to review care and progress.
  • Ensure compliance with NCQA requirements and utilization management processes.
  • Contribute to data collection, care gap closure, and quality metrics.
  • Maintain up-to-date knowledge of company products and services.
  • Manage caseload efficiently, ensuring accurate documentation and quality audit scores.
  • Identify opportunities for process improvement in patient care.

Required Qualifications:
  • Current, unrestricted Compact RN license.
  • At least 3 years of clinical experience with adults and/or geriatric populations.
Preferred Qualifications:
  • Bachelor's Degree in Nursing.
  • Certification in Case Management (CCM).
  • Experience with complex case management, DSNP NCQA requirements, managed care, or chronic condition management.

Remote employees must adhere to UnitedHealth Group's Telecommuter Policy.

The salary range is $59,500 to $116,600 annually, based on various factors including experience, education, and location. Benefits include a comprehensive package, incentives, stock purchase options, and 401k contributions, subject to eligibility.
