Responsibilities

  • Perform in-depth assessments of patients’ medical, psychosocial, and financial needs upon admission.
  • Identify barriers to discharge (transportation, home support, insurance, medication access).
  • Develop individualized care and discharge plans with the interdisciplinary team.
  • Coordinate with physicians, nurses, therapists, and external providers to ensure cohesive care delivery.
  • Document all case management activities for compliance and quality reporting.
  • Educate patients and families on treatment plans, medications, and post-discharge instructions.

POSITION SUMMARY:

Adhering to S ystem Values, Service Excellence , and Standards of B ehavior , the Inpatient Case Manager leads comprehensive patient care coordination during hospitalization, ensuring safe, timely , and effective discharge while addressing medical, psychosocial, and financial barriers. This role requires clinical judgment, decision-making, and collaboration with interdisciplinary teams to optimize patient outcomes and continuity of care, especially in resource-limited rural settings.

The anticipated hourly range for this position is$27.27/hr - $34.10/hr - $36.83/hr. Final compensation will be determined based on the selected candidate's applicable experience, skills, qualifications, and other job-related factors.

ESSENTIAL QUALIFICATIONS

Education:

  • A Bachelor's in Social Work or Human Services, or a related field is required .

Certification:

  • Certification in Case Management (CCM) or equivalent is preferred.
  • All employees and contracted staff will have either successful completion of an American Heart Association* accredited course in Basic Life Support (BLS) , or ACLS and PALS combined. (for clinical staff), Heartsaver CPR AED (for nonclinical staff), and renewal on a regular basis .

ESSENTIAL FUNCTIONS :

  • Perform in-depth assessments of patients’ medical, psychosocial, and financial needs upon admission.
  • Identify potential barriers to discharge (transportation, home support, insurance , medication access).
  • Develop individualized care and discharge plans in collaboration with the interdisciplinary team.
  • Clinical Decision-Making & Coordination
  • Determine appropriate level of care for patients (inpatient, observation, rehabilitation, home health, skilled nursing ).
  • Coordinate with physicians, nurses, therapists, and external providers to ensure cohesive care delivery.
  • Make recommendations to facilitate safe transitions and prevent readmissions.
  • Resource Navigation & Advocacy
  • Connect patients and families with community resources, financial assistance programs, and post-acute care services.
  • Advocate for patient needs to ensure access to appropriate services, including rural or hard-to-access facilities.
  • Participate in utilization review and ensure care aligns with payer and regulatory requirements.
  • Document all case management activities accurately for compliance and quality reporting.
  • Patient & Family Education
  • Educate patients and caregivers on treatment plans, medications, post-discharge instructions, and follow-up care.
  • Provide coaching to support adherence and self-management of health conditions.
  • P erforms additional duties as assigned.
