Job summary

UCHealth Memorial Hospital Central in Colorado Springs is seeking a Transition of Care Social Worker to organize and coordinate care for patients with complex needs, reduce length of stay, lower readmissions, and ensure safe dispositions. The role is onsite, full-time, days.

Requires a Master’s degree in Social Work and LCSW licensure. You will collaborate with care management and social work teams and mentor others in complex discharge planning.

Qualifications

• Master's degree in Social Work is required.
• Licensed Clinical Social Worker (LCSW) licensure is required.

Responsibilities

• Assesses, plans, coordinates, monitors and evaluates the options and services required to transition patients with complex care needs from hospital to the community.
• Uses resources to address complex care needs including guardianship, foreign nationals and bariatric patients with significant post-acute needs, history of behavioral or substance abuse.
• Acts as a resource to Social Work and Care Management teams, mentoring around skilled communication and resources for complex patient care situations.
• Collaborates with managers to develop innovative options for complex patients and may participate in leadership meetings regarding complex care transitions.
