Job summary

Qualifications

• MSW from an accredited School of Social Work.
• Licensed by the State of Ohio Social Work Board (LSW, LISW, or LISW-S).
• NCI-APS (Nonviolent Crisis Intervention with Advanced Physical Skills) Certification must be obtained within 90 days of hire for employees in UC Health Psychiatric Services.
• Minimum 1–2 years of experience; 3–5 years preferred.

Responsibilities

• Respond to referrals from the interdisciplinary team and engage patients and families.
• Complete psychosocial assessments and develop a plan of care addressing patients' needs.
• Facilitate transitions to the next level of care and coordinate resources.
• Provide education and consultation to the healthcare team on psychosocial, emotional, financial, and developmental aspects of care.
• Advocate, mediate and collaborate to ensure cohesive care plans and safe dispositions.
