Job summary

UCHealth Memorial Hospital Central in Colorado Springs is seeking a Care Management professional to organize and coordinate transitions for patients with complex care needs, reducing length of stay and ensuring safe dispositions.

The role is onsite with a full-time schedule (80 hours per pay period) and a shift of days. A Master’s in Social Work and LCSW licensure are required to practice in this position.

Qualifications

• Master's degree in Social Work is required.
• State licensure as a Licensed Clinical Social Worker (LCSW) is required.

Responsibilities

• Coordinate care transitions for patients with complex needs from hospital to community.
• Utilize resources to address complex care needs (e.g., guardianship, housing, behavioral health).
• Serve as a resource to Social Work and Care Management teams with mentorship and guidance on communication, education, and resources.
• Collaborate with managers to develop options for complex patients; participate in leadership discussions as needed.
• Demonstrate critical thinking, decisive judgment, and the ability to work with minimal supervision in a fast-paced environment.
