Job summary

Mass General Brigham Community Physicians, Inc. is seeking a Social Worker to provide comprehensive psychosocial assessments and interventions for inpatients, outpatients, and ED patients. The role emphasizes patient- and family-centered care within a multidisciplinary team.

The position is full-time with a hybrid schedule (in-office and remote work) across Harbor Medical Practices and Pembroke/S. Weymouth locations.

Qualifications

• Master's Degree in Mental Health & Behavioral Medicine or Master’s Degree in Social Work required.
• Driver's License required.
• Licensed Independent Clinical Social Worker (LICSW) or Licensed Mental Health Counselor (LMHC) required.
• 2+ years clinical experience in a medical setting required.
• Case management experience preferred.

Responsibilities

• Performs psychosocial assessments of inpatients, outpatients, and ED patients.
• Develops biopsychosocial assessment, disposition, and treatment plans.
• Collaborates with interdisciplinary team to evaluate patients' medical condition and needs.
• Monitors, evaluates, and documents client progress per treatment plan.
