Job summary

Monarch Communities in Colts Neck Township, New Jersey, is seeking a Licensed Practical Nurse (LPN) to provide holistic nursing care to residents under the direction of the Health and Wellness Director. The role emphasizes compassionate care and supervision of Certified Nursing Assistants.

The Wellness Nurse works full-time across 7am–3pm and/or 3pm–11pm shifts, focusing on health, safety, and quality of life for residents, while supporting regulatory compliance and care planning.

Qualifications

• Graduation from an accredited School of Nursing, with current LPN license
• At least 2 years' experience with long-term care, assisted living, home health or hospital setting preferred
• Experience working with residents with Alzheimer's or other dementias
• Ability to handle multiple tasks
• Knowledge of federal and state regulations, nursing practices and health and wellness procedures
• Proficiency in computer skills (MS Office) with ability to learn new applications
• Physical requirements including standing, walking, sitting, lifting up to 50 lbs

Responsibilities

• Monitors the health, safety, and well-being of all residents
• Assist in training and monitoring medication administration
• Supervise care staff in accordance with state regulations and community policies
• Maintain clinical quality assurance per federal, state, and local standards
• Promote high-quality service and embody the company mission
• Conduct thorough resident assessments
