Job summary

Diversicare Healthcare Services, LLC is seeking a dedicated Certified Nursing Assistant (C.N.A.) to join our team at our Tennessee facilities. The role focuses on resident safety during transport and visits, with a strong emphasis on compassionate care and service excellence.

Qualifications

• Must be a Certified Nurse Assistant (C.N.A.) with valid state certification.
• Ability to transfer residents and maintain safety.
• Clean driving record and valid state driver’s license.
• No disqualifying criminal record.

Responsibilities

• Assure the safety of each resident when transporting to and from the doctor’s office and/or hospital.
• Accompany each resident on all medical visits. Stay with resident(s) until visit is complete.
• Observe the following guidelines at each visit.
• Sign in resident upon entrance.
• Present resident’s documents to office personnel.
• Before leaving doctor’s office, please observe the following:
• Obtain resident’s documents.
• Obtain any new prescriptions.
• Check to make sure doctor has written on the doctor’s order form.
• If follow-up appointment necessary, obtain appointment cards.
• After returning to facility, report to RCM and be prepared to answer questions concerning visit.
• Performs other duties as required and assigned.
