Job summary

Qualifications

• Must be a Certified Nurse Assistant (C.N.A.).
• Must possess a valid state driver’s license.
• Clean driving record and clean criminal history are required.

Responsibilities

• Ensure resident safety during transport to and from medical appointments.
• Accompany residents to doctor’s visits and stay with them until visits are completed.
• Obtain and verify resident documents and prescriptions after appointments.
• Return to facility and report visit details to the Regional Care Manager.
