Job summary

Qualifications

• Must be a Certified Nurse Assistant (C.N.A.).
• Physically able to assist ambulatory and non-ambulatory residents.
• Clean driving record.
• Valid state driver’s license.
• Any other state required certifications

Responsibilities

• Assure the safety of each resident during transport to visits.
• Accompany residents to medical visits and stay until complete.
• Document and present resident’s documents to office staff.
• Report results to the RCM after visits.
• Perform other duties as assigned.
