Qualifications

• Must be physically able to assist residents with transfers.
• Must possess a valid state driver's license.
• Certified Nurse Assistant (CNA) required.
• Clean driving record and clean criminal record.

Responsibilities

• Ensure resident safety during transport to medical visits.
• Accompany residents on medical visits and stay with them until finished.
• Present resident documents to office personnel.
• Obtain resident documents and any new prescriptions after visits.
• Check doctor’s orders and obtain appointment cards if needed.
• Report outcomes to the RCM after returning to the facility.
• Perform other duties as required.
