Job summary

Diversicare is seeking a dedicated CNA to assist residents with mobility and daily care, ensuring safe transportation to medical appointments and during visits. The role emphasizes compassionate patient care and clear communication with the care team.

Candidates must be a Certified Nurse Assistant with a valid state license, able to pass background checks, and committed to a high standard of service in a skilled nursing facility setting.

Qualifications

• C.N.A. certification required and active.
• Strong patient-care and mobility-support skills.
• Good communication and observation abilities.

Responsibilities

• Assure resident safety during transport to/from doctor or hospital.
• Accompany residents to medical visits and stay with them until completion.
• Sign in residents and present documents at visits; obtain necessary papers.
• Collect and secure resident documents and prescriptions after visits.
• Report to RCM and answer questions regarding visits; perform other duties as assigned.
