The position covers the Lexington and Frankfort regions, ensuring reliable service for PACE participants and adherence to program guidelines.

Qualifications

• Must possess a valid Kentucky driver’s license.
• CPR certification required.
• Strong reading and writing comprehension.
• Demonstrated ability to interact with participants, caregivers and staff.
• Able to keep accurate records and use basic computer tools.

Responsibilities

• Transport PACE participants to and from destinations (home, PACE Center, medical providers, etc.).
• Maintain professional interaction with participants, caregivers, community agencies and PACE staff.
• Ensure safety and timely transportation while documenting trips.
