Responsibilities

• Assist in planning, developing, organizing, implementing, evaluating, and directing social service programs.
• Meet with administration and staff to plan social services.
• Assist in identifying medically related social and emotional needs.
• Participate in community planning related to resident services.
• Complete forms and reports for the Director.
• Involve residents/families in planning when possible.
• Coordinate activities with departments as needed.
• Provide information about Medicare/Medicaid and financial assistance.
• Assist with discharge planning and referrals to community agencies.
