Job summary

Milestone is seeking compassionate Care Partners to support residents in our Assisted Living and Memory Care communities. You will provide personal care, support meals and dining, and communicate important changes in residents’ conditions to the Health & Wellness team.

This full-time role requires PM shifts and weekend coverage. Join a welcoming team dedicated to safe, high-quality, individualized care, teamwork, and strong relationships with residents and families.

Qualifications

• High School Diploma or equivalent.
• Strong interpersonal, verbal, and written communication skills.
• Ability to communicate clearly in English and complete documentation accurately.
• Compassion for and desire to work with older adults.

Responsibilities

• Follow each resident’s service plan in a friendly, respectful, and supportive manner.
• Document changes in resident condition per company policy and state requirements.
• Support Continuous Quality Improvement by identifying issues and communicating them to leadership.
• Adhere to all company policies and regulations including HIPAA, OSHA, and infection control guidelines.
• Promote a safe working environment and report unsafe actions immediately.
• Report any suspected elder maltreatment or abuse according to policy.
• Communicate effectively with fellow Care Partners regarding resident needs.
• Assist with meal service including serving, clearing, and dining room cleanliness.
• Foster teamwork, respect, and positive interactions with residents, families, and coworkers.
• Perform other duties as assigned.
