Job summary

Milestone is seeking compassionate Care Partners to support residents in Assisted Living and Memory Care communities. You will provide personal care, assist meals, and relay important health changes to the Health & Wellness team, ensuring safe, high-quality, individualized care daily.

The role emphasizes compassionate service, strong communication, and teamwork to enhance resident well-being and family satisfaction. Flexible shifts and adherence to policy are expected.

Qualifications

• High School Diploma or equivalent required.
• At least one year of caregiving or similar experience.
• Strong interpersonal, verbal, and written communication skills.
• Ability to communicate clearly in English and complete documentation accurately.
• Certifications as required by state.

Responsibilities

• Follow each resident’s service plan in a friendly, respectful, and supportive manner.
• Document changes in resident condition per company policy and state requirements.
• Support Continuous Quality Improvement by identifying issues and communicating them to leadership.
• Adhere to all company policies and regulations including HIPAA, OSHA, and infection control guidelines.
• Promote a safe working environment and report unsafe actions immediately.
• Report any suspected elder maltreatment or abuse according to policy.
• Communicate effectively with fellow Care Partners regarding resident needs.
• Assist with meal service including serving, clearing, and dining room cleanliness.
• Foster teamwork, respect, and positive interactions with residents, families, and coworkers.
• Perform other duties as assigned.
