WelbeHealth is seeking a Personal Care Assistant to join our Long Beach day center, delivering hands-on personal care and supporting participants with daily living activities. You will be part of the interdisciplinary team, contributing to safe, compassionate care and participates in care planning.

The role involves assisting with meals, documenting vital signs, transporting participants, and maintaining a clean, safe day-center environment in a value-based model. Vaccination policy applies.

Qualifications

• High school diploma or equivalent required.
• Certification of Home Health Aide (HHA) or Certified Nursing Assistant in California (CNA) preferred.
• Minimum of one (1) year of experience providing personal care to seniors.
• Must be willing and able to drive to/from participant locations within the program service area.
• CA Food Handlers Certification within 30 days of hire.
• CPR and First Aid certification within 180 days of hire and maintain certification thereafter; CPR/First Aid certification at hire preferred.

Responsabilités

• Provide hands-on personal care assistance with ADLs to participants, including escorting, toileting, bathing, dressing, grooming, laundry, and lifting.
• Measure and document participants’ vital signs as instructed.
• Act as medical escort when needed, assisting participants with WelbeHealth transportation.
• Oversee activity on the day center floor to ensure participant safety.
• Assist with meal service and feeding when needed.
• Maintain personal care supplies and grooming items.
