Job summary

Caremed, Inc. is seeking compassionate Registered Nurses (RN) to provide high‑quality, personalized home health care across Virginia.

Roles include Full‑Time or PRN positions served across Appomattox, Charlotte, Campbell, Lunenburg, and Mecklenburg Counties. You will perform home visits, assess needs, educate patients and families, and collaborate with the care team to ensure continuous, patient‑centered care.

Qualifications

• Active RN license in good standing (Virginia)
• Graduate of an accredited nursing program (ADN or BSN)
• Active CPR / BLS certification
• 1+ years of home health or home care experience preferred
• Valid driver’s license, reliable vehicle, and active auto insurance

Responsibilities

• Personalized Home Visits: Perform health evaluations and deliver individualized care in patient homes.
• Holistic Assessments: Evaluate physical, psycho-social, and home environment factors to tailor care plans.
• Empower & Educate: Teach patients and family caregivers how to manage health conditions effectively.
• Team Collaboration: Work alongside multidisciplinary teams to ensure complete, continuous care.
• Documentation & Leadership: Complete timely visit reports and provide supportive guidance to home care aides.
