LionStone Care is seeking a Registered Nurse (RN) for the Shaker Gardens Care Community in Shaker Heights, OH. The role focuses on direct patient care in a long-term/assisted living setting, with emphasis on safety, compassion, and quality outcomes.

Responsibilities include assessments, developing care plans, administering medications, documenting thoroughly, and partnering with the care team to ensure continuity. Second shift, full-time, with competitive hourly pay.

Qualifications

• Graduate of an accredited nursing program (Associate’s or Bachelor’s).
• Active RN license required.
• New graduates welcome; healthcare or long-term care experience preferred.

Responsibilities

• Conduct comprehensive assessments of patient health status.
• Develop and implement individualized care plans with the team.
• Administer medications and treatments per orders and policies.
• Maintain accurate documentation in the EHR and ensure compliance.
• Collaborate with physicians and interdisciplinary team for coordinated care.
• Respond to medical emergencies and provide timely interventions.
• Advocate for patients and families and support care conferences.
