Responsibilities

• Assess patient/resident health status and develop care plans with the team.
• Administer medications and treatments per orders and policy.
• Monitor patient responses and adjust plans as needed; document promptly.
• Perform wound care, IV therapy, catheter care, and respiratory treatments as required.
• Provide patient/family education on medications and care plans.
• Maintain accurate documentation in the EHR and ensure regulatory compliance.
• Coordinate with physicians and interdisciplinary team for safe, effective care.
