Job summary

LionStone Care in Shaker Heights, OH is seeking an LPN for full-time, second shift in a long-term care setting. Under the direction of RNs and physicians, you will administer medications, monitor residents, and contribute to individualized care plans.

You will collaborate with a multidisciplinary team to support daily living activities, maintain health records, and communicate with families. This role emphasizes compassionate, patient-centered care and safety.

Qualifications

• Graduate of an accredited LPN program.
• Active LPN license.
• Experience in healthcare or long-term care preferred but not required.

Responsibilities

• Provide basic nursing care including medications, wound care, and daily activities.
• Monitor and document vital signs and report changes to the healthcare team.
• Administer prescribed medications with correct dosages and timings.
• Develop and implement individualized care plans with RNs and physicians.
• Assist with clinical procedures and ensure a clean, safe work environment.
