Job summary

A Path of Care Hospice in Weatherford, OK is seeking a full-time RN Case Manager to provide compassionate, high-quality nursing care to patients in their homes. Qualifications

• Active Oklahoma Registered Nurse license.
• At least 1 year of clinical experience.
• Reliable transportation and vehicle liability insurance.
• Hospice, home health, critical care, or emergency room experience preferred.
• Strong interpersonal and communication skills.
• Ability to mentor others with compassion and patience.
• Commitment to integrity, critical thinking, and teamwork.

Responsibilities

• Complete initial assessments within 48 hours of hospice election.
• Work with attending physician and interdisciplinary team to complete comprehensive assessments.
• Develop, coordinate, and initiate individualized plans of care.
• Conduct ongoing assessments and updating care plans based on changes in patient condition.
• Address physical, psychological, social, and spiritual needs.
• Communicate regularly with physicians and the care team to maintain continuity of care.
• Maintain accurate, timely documentation to meet regulatory requirements.
• Provide leadership by instructing caregivers and volunteers and supervising LPNs and hospice aides.
• Practicing infection control and reporting any signs of abuse.
• Provide direct patient care as needed and assisting with discharge planning.
• Participating in professional development, community programs, and agency initiatives.
