Job summary

A Path of Care Hospice in Weatherford, OK is seeking a full‑time RN Case Manager to provide compassionate, high‑quality nursing care to patients in their homes. Qualifications

• Active Oklahoma Registered Nurse license.
• At least 1 year of clinical experience.
• Reliable transportation and vehicle liability insurance.
• Hospice, home health, critical care, or emergency room experience preferred.
• Strong interpersonal and communication skills.
• Ability to adapt to changing circumstances, mentor others, and work with compassion and patience.
• Commitment to integrity, critical thinking, and teamwork.

Responsibilities

• Perform initial assessments within 48 hours of hospice election.
• Collaborate with attending physicians and the interdisciplinary team to complete assessments.
• Develop, coordinate, and initiate individualized plans of care.
• Continuously reassess and update care plans as patient conditions change.
• Address physical, psychological, social, and spiritual needs of patients and families.
• Communicate regularly with physicians and care team to ensure continuity of care.
• Maintain accurate and timely documentation to meet regulatory requirements.
• Provide leadership by instructing caregivers and supervising LPNs and aides.
• Practice infection control and report signs of abuse.
• Contribute to discharge planning and direct patient care as needed.
• Engage in professional development and agency initiatives.
