Job summary

All Heart Home Care in the Nashville area seeks an experienced Home Health Case Manager (RN) to coordinate and manage patient care from admission through discharge in patients' homes. The role offers full-time scheduling and requires Tennessee RN licensure with a strong emphasis on documentation and communication.

The successful candidate will conduct patient visits as needed, manage orders and PCC processes, and collaborate with clinicians to ensure care transitions and regulatory compliance.

Qualifications

• Active Tennessee RN license or Compact State licensure.
• BLS/CPR certification required.
• Two years of home health experience preferred.
• Strong documentation and communication skills.

Responsibilities

• Coordinate patient care from admission through discharge.
• Prepare PCC recertification and discharge forms.
• Order and manage physician orders and DME.
• Maintain EMR documentation and patient charts.
• Collaborate with clinicians on discharge planning and care transitions.
