Heart'n Soul Hospice is seeking an RN Case Manager in Nashville to conduct comprehensive assessments, develop and revise care plans, and coordinate multidisciplinary care for patients and families facing life-limiting illness.

Qualifications

• Graduate of an accredited Diploma, Associate or Baccalaureate School of Nursing.
• Current State Registered Nurse License.
• Current Driver's License, reliable transportation, and provide ongoing valid and current auto liability insurance.
• Two years of experience as an RN preferred in a healthcare/clinical setting.

Responsibilities

• Initial and ongoing comprehensive assessments of the impact of terminal disease on the patient’s physical, psychosocial, and environmental needs and ADLs.
• Implement the individualized POC and revise the plan as necessary.
• Collaborates care among patient/patient caregiver, physicians and healthcare professionals.
• Attend Inter-Disciplinary Group (IDG) meeting and coordinate patient care planning with IDG.
• Perform on-call responsibilities and provide on-call service to patients/families.
