North Kansas City Hospital Home Health, LLC seeks a Registered Nurse Case Manager to lead patient assessment and care planning in the home health setting. The RNCM coordinates care across disciplines under physician direction and educates patients and families while ensuring safe, compliant, and cost-effective service.

The role requires supervising care delivery, completing timely documentation, and staying current with state regulations. Travel to patient locations is required as scheduled.

Qualificações

• Minimum of one year of RN experience in an acute care setting.
• Home care experience is desired.
• Excellent communication with patients, families, physicians and coworkers.
• Calm, patient, compassionate, and team-oriented.
• Knowledgeable of nursing best practices and sound clinical judgment.
• Ability to follow directions and adhere to regulatory requirements.

Responsabilidades

• Oversees the overall care of the patient from beginning to end of episode
• Updates doctor orders in patient chart accordingly
• Notifies doctor of significant changes in patient condition
• Manages multidisciplinary care as applicable
• Provides skilled nursing care in the patient’s home as ordered
• Completes clinical documentation per agency protocols and Medicare guidelines
• Maintains active RN license and drives to patient locations as scheduled
• Participates in on-call requirements and case conferences
