Job summary

Absolute Hospice in Canton, OH is seeking an experienced RN to join our hospice team as a Hospice RN Case Manager. You will provide direct patient care and support the interdisciplinary group to deliver high-quality hospice services.

You will assess patients, develop and implement the plan of care, educate patients and families on medications, dressing changes, oxygen and hospice care, and manage patient clinical records to ensure accurate documentation.

Qualifications

• Active RN license and hospice care experience.
• Ability to assess patients and develop plan of care.
• Educate patients and families on medications, oxygen, and end-of-life care.

Responsibilities

• Assess patient needs and document in the plan of care.
• Develop and implement hospice plan of care with the care team.
• Educate patients and families on medications, dressing changes, oxygen, and hospice care.
