Job summary

Cherish Hospice in Greenville, OH is seeking a compassionate Hospice RN Case Manager to coordinate patient-centered care in home and facility settings. You will collaborate with a multidisciplinary team to tailor plans that enhance comfort and quality of life for patients and families.

You will travel within Miami and Darke counties to deliver in-home care, with a focus on advocacy, counseling, and consistent documentation of patient progress.

Qualifications

• Active RN license is required.
• Valid driver's license required.
• Minimum 1 year hospice experience.
• BLS certification preferred.
• Hospice and palliative medicine background preferred.

Responsibilities

• Lead with heart by guiding a multidisciplinary team to create personalized care experiences.
• Advocate for each patient's needs, while supporting families.
• Provide counseling and emotional support to patients and loved ones.
• Develop and implement care plans that prioritize quality of life and comfort.
• Document patient journeys and track progress and milestones.
