Qualifications

• Must be at least 18 years of age.
• High School Diploma or GED required.
• Valid Driver's License required.

Responsibilities

• Provide transportation for individuals to medical appointments, community activities, school/day programs, and other approved activities.
• Support individuals in achieving goals outlined in their ISP and BSP.
• Follow supervision requirements and support strategies identified in individualized plans.
• Promote independence, choice, dignity, and community inclusion.
• Maintain a safe, clean, and supportive living environment for individuals served.
• Accurately complete required documentation (logs, progress notes, incident reports, and related records).
• Attend and participate in scheduled home and office staff meetings; assist with on-call duties as needed.
