Responsibilities

• Provide direct care to residents and report changes in condition to the Resident Care Director.
• Assist residents with lifts, moves, and transports with proper body mechanics.
• Respond to inquiries from residents, visitors, and staff promptly and courteously.
• Follow individualized care plans and contribute observations to the care planning process.
• Communicate effectively with residents, families, peers, and supervisors to promote a homelike environment.
• Maintain confidentiality of Protected Health Information and report any policy non-compliance.
