Qualifications

• Bachelor’s degree in social work or related field or an associate’s degree with related experience.
• Experience in providing services to older adults is preferred.
• Knowledge of care planning and resident support is desirable.

Responsibilities

• Assist with admission of residents and provide case management services.
• Serve as the discharge planner for the facility.
• Coordinate services and entitlements from public and private sources.
• Maintain records and documentation of case management activities.
• Arrange transportation and coordinate with staff for resident needs.
• Support residents in obtaining medical care and primary physician contact.
