Job description

Case Manager

We are seeking a compassionate and dedicated case manager to join our growing team. You will coordinate resident services and commit to resident satisfaction and quality of care in a supportive team of healthcare professionals.

What You'll Do

• Uphold and maintain adherence to residents’ Bill of Rights
• Serve as a key point of contact between residents, families, and care providers
• Conduct assessments and coordinate admissions and re‑admissions from the community, hospitals, and nursing homes
• Coordinate and supervise clinical care for residents across the continuum
• Maintain accurate, clear, and timely documentation in ALIS in accordance with facility standards

What We're Looking For

• Minimum 1 year of experience as a Case Manager required; long‑term care experience is a strong plus
• Experience with ALIS or a similar EMR system is a plus
• Strong communication skills with the ability to work effectively with families and outside care providers
• Highly organized with a genuine passion for working with the senior population
• Team player who thrives in a multidisciplinary environment
