Job summary

Mountainside Treatment Center in Canaan, CT is seeking a compassionate Recovery Coach to support clients in defining and achieving personal recovery goals. You will develop individualized wellness plans and provide ongoing guidance, acting as advocate and mentor within our Connected Care Network.

The role involves documenting progress, coordinating with clinicians, and helping clients transition back to work or school.

Qualifications

• Strong communication and relationship-building skills.
• Experience in recovery coaching or substance use treatment.
• 1–2+ years in substance use recovery settings.

Responsibilities

• Provide one year or more of individualized support, education, and advocacy to clients and families.
• Assist clients in locating personal and recovery resources (AA, NA, Al‑Anon, therapists).
• Facilitate transitions back to work, school, or home while supporting Mountainside’s continuing care plan.
• Educate and support families around communication, boundaries, and self-care.
• Schedule and document random toxicology screenings.
• Develop and monitor individualized Wellness Plans and recovery pathways.
• Refer clients to clinical, medical, legal, or social services as needed.
• Monitor client progress and adapt support strategies accordingly.
• Maintain accurate documentation and timely reports in the EMR system.
