Job summary

Silver Birch Living in Fort Wayne is seeking a compassionate and dependable Qualified Medication Assistant (QMA) to join our Clinical Team. You will assist with medications, daily care, and support residents' health and safety.

You will document care in the electronic health record, respond to call lights, and work closely with the Wellness team to meet resident needs. Requires a QMA certification; high school diploma or GED; insulin cert preferred.

Qualifications

• Must hold a current QMA certification.
• High school diploma or GED required.
• Insulin certification preferred or willingness to obtain.
• Healthcare or senior living experience is helpful.
• Comfort using electronic documentation systems (EHR).

Responsibilities

• Assist residents with ADLs and daily care following individualized care plans.
• Administer, monitor, and document medications and treatments.
• Observe residents and report changes to Wellness Nurse and clinical team.
• Document care and services accurately in the electronic health record.
• Respond promptly to call lights and resident requests.
• Provide friendly, respectful customer service to residents, families, and team members.
• Collaborate with the Wellness team to support clinical operations.
