Silver Birch Living in Fort Wayne is seeking a compassionate Qualified Medication Assistant (QMA) to join our clinical team. You will support residents’ health, safety, and daily care needs by assisting with medications and personal care, ensuring residents feel cared for, respected, and supported each day.

The role emphasizes teamwork, adherence to individualized care plans, and accurate documentation in the electronic health record, with opportunities for growth within a supportive workplace.

Qualifications

• High school diploma or GED.
• Current QMA certification in good standing.
• Insulin certification preferred, or willingness to obtain.
• Ability to maintain required training and certifications.
• Compassionate, resident-centered approach to care.
• Strong communication and teamwork.
• Experience in healthcare or senior living is helpful.

Responsibilities

• Assist residents with ADLs and daily care per care plans.
• Administer, monitor, and document medications as directed.
• Observe residents and report changes to Wellness Nurse.
• Document care in the electronic health record.
• Respond to call lights and resident requests.
• Provide friendly, respectful service to residents, families, and team.
• Support clinical operations with Wellness team.
