Job summary

Good Samaritan Hospice is seeking a compassionate Registered Nurse – PRN to provide expert clinical care and support our nursing teams across home care and inpatient services in Virginia. You will participate with the Interdisciplinary Team to ensure holistic, patient- and family-centered care, including admissions and emergent visits.

This flexible PRN role emphasizes meaningful end-of-life care, with a schedule that varies by patient and staffing needs.

Qualifications

• Graduate of an accredited nursing education program.
• Minimum of two years of nursing experience.
• Currently licensed as a Registered Nurse in Virginia.
• Experience in hospice or home health for home care assignments; inpatient requires acute care, home health, hospice, medical-surgical, or geriatric nursing.

Responsibilities

• Participate as a member of the Interdisciplinary Team (IDT) to provide holistic, patient- and family-centered care.
• Provide nursing visits on a scheduled or emergent basis based on patient and staffing needs.
• Complete comprehensive nursing assessments including pain, nutrition, skin integrity, and changes in condition.
• Complete timely admission assessments within required timeframes.
• Develop collaborative care plans with patients, families, and IDT and contribute to ongoing care planning.
• Participate in IDT care conferences and communicate patient needs to team members.
• Provide education to promote independence and dignity at end of life, and support caregivers.
• Obtain physician orders and document verbal orders per policy.
• Maintain accurate EMR documentation and respond to families at patient death with bereavement support.
• Provide coverage for staffing and admissions and float between home care and inpatient services.
