Job summary
Good Samaritan Hospice is seeking a compassionate Registered Nurse Case Manager to provide expert clinical care, guidance, and comfort during end-of-life journeys in the New River Valley area. You will serve on the Interdisciplinary Team, conducting assessments, admissions, and ongoing evaluations to ensure patient- and family-centered care.
The role requires an active Virginia RN license, at least two years of nursing experience (hospice/home health preferred), strong clinical judgment,
Qualifications
- Licensed RN in Virginia
- Minimum 2 years nursing experience
- Hospice or home health experience preferred
- Proficient with EMR documentation
- Valid driver’s license and reliable transportation
Responsibilities
- Provide holistic, patient- and family-centered hospice care as part of the Interdisciplinary Team
- Perform comprehensive nursing assessments, admissions, and ongoing reassessments per hospice regulations
- Develop and evaluate individualized care plans with IDT and attending physicians
- Deliver skilled nursing care including symptom management and pain control
- Educate patients and families on disease progression and caregiving tasks
- Offer emotional support and compassionate guidance throughout the hospice journey
- Fulfill on-call duties and assist with admissions as needed
