Qualifications

• Graduate of an accredited school of nursing with a current state license as a registered nurse.
• Minimum 2+ years of nursing experience in a clinical care setting; hospice experience preferred.
• Reliable transportation required.

Responsibilities

• Evaluate patients and create care plans.
• Communicate with the patient, family, and the care team to coordinate services.
• Drive care team to ensure every patient receives quality hospice care.
