• Ability to read, interpret medical information.
• Ability to collaborate with residents, families, and healthcare professionals.

Responsibilities

• Evaluate and document evaluations of referred residents.
• Provide appropriate treatment to help residents regain function.
• Recommend adaptive equipment and preventative measures.
• Educate residents and caregivers to maximize function.
• Maintain timely resident reports per regulations.
• Complete documentation of evaluations, plans, and progress notes.
• Participate in interdisciplinary conferences and care planning.
• Follow facility policies and safety guidelines.
