Qualifications

• Ability to read, analyze, and interpret common medical information; communicate with residents, families and healthcare professionals.
• Proficient with Word and Excel.

Responsibilities

• Evaluate and document evaluations of referred residents per facility policies and procedures.
• Provide treatment to help residents maximize function within their physical abilities.
• Recommend and/or provide appropriate treatment and adaptive equipment to prevent decline in function.
• Provide resident or caregiver education to maintain or improve function.
• Maintain timely and accurate resident reports for regulators and third-party payers.
