Job summary

Abbotsford Health Care Center is seeking an Occupational Therapist to join our skilled nursing team in Abbotsford, WI. The role focuses on evaluating and treating residents to maximize independence while collaborating with interdisciplinary staff.

The ideal candidate holds an MS in Occupational Therapy with NBCOT certification, demonstrates strong communication, and participates in care planning and education for residents and families.

Qualifications

• Excellent verbal and written communication and organizational skills.
• Ability to read, analyze, and interpret common medical information.
• Ability to effectively communicate with residents, their families and other healthcare professionals.
• Ability to present information to supervisor.

Responsibilities

• Evaluate residents and document evaluations per facility policies.
• Provide treatment to maximize function within physical abilities.
• Recommend adaptive equipment to prevent decline.
• Educate residents or caregivers to maintain highest function.
• Maintain timely resident reports for regulatory agencies and payers.
• Attend care conferences and rehabilitation conferences as scheduled.
