Job summary

Carle Health seeks a licensed Physical Therapist to evaluate and treat patients of all ages, updating plans of care and coordinating discharge plans. You will educate patients and families, document outcomes, and collaborate with physicians to optimize rehabilitation results.

The role requires a PT license, BLS within 30 days, and a Bachelor’s Degree in Physical Therapy, with experience in orthopedic/ neurologic conditions and evidence-based practice.

Qualifications

• BLS certification within 30 days of hire.
• Licensed Physical Therapist (PT) in good standing.
• Bachelor’s Degree in Physical Therapy (Required).

Responsibilities

• Demonstrates ability to perform patient transfers with assistance as needed.
• Uses safe body mechanics to reduce risk of injury to self, staff and patients.
• Designs a plan of care with goals, treatment, outcomes, and discharge plan.
• Educates patients, family, caregivers, students, and other health care providers.
• Evaluates clinical findings to determine PT diagnoses and outcomes.
• Produces documentation in EMR and communicates with referring providers.
