Qualifications

• BLS certification must be obtained within 30 days of hire.
• Current Physical Therapist license (IDFPR) required.

Responsibilities

• Demonstrates ability to perform patient transfers with assistance as needed.
• Ensures safe body mechanics to reduce injury risk to self, staff and patients.
• Designs an appropriate plan of care integrating goals, treatment, outcomes, and discharge plan.
• Educates patients, family, caregivers, and other health care providers using effective teaching methods.
• Evaluates clinical findings to determine physical therapy diagnoses and outcomes of care.
• Performs examination and selects reliable methods relevant to chief complaint and history.
• Maintains documentation in EMR per CTSCP67 policy.
• Communicates progress and recommendations with referring providers.
