Job summary

• Two years of experience as a physical therapist or in home health preferred.
• CPR certification current and valid.
• Licensed to practice physical therapy in California.

• Provides PT services per physician plan of care and evaluates functional status.
• Develops and implements care plans and documents patient progress.
• Educates patients and families on treatment and home exercise programs.
• Coordinates discharge planning and referrals for additional services.
