ABL Health Care in Concord, CA is seeking a Full Time Physical Therapist (PT) to join our Home Health team. You will evaluate patients, develop and implement care plans, and monitor progress in patients' homes, collaborating with our Clinical Director and staff.

The role requires a California PT license, at least 1 year in home health or hospice, a valid CA driver’s license, and willingness to travel within a 15–25 mile radius. We offer comprehensive benefits and a supportive back office.

Qualificações

• Current PT License to practice in California - Required
• Experience in Home Health or Hospice care - 1 Year Required
• Current CA Driver’s License
• Able to travel within a 15 - 25 mile radius on a regular basis.

Responsabilidades

• Provide, coordinate and oversee skilled intermittent care to patients and include instruction and education of patients and/or caregivers to promote maximal functional status, in the patient’s home.
• Regularly evaluate and re‑evaluate patient status.
• Administer treatments, monitor patient conditions, and report signs/symptoms indicating a change in patient condition to Physician, Case Manager, and/or Director of Nursing as needed.
• Initiate, develop, implement and revise the patient’s plan of care (POC).
• Address all problems identified in the POC.
• Provide educational information to the patient/family/caregiver concerning knowledge deficits or needs regarding the patient’s condition.
• Communicate with other disciplines involved in the case as warranted.
• Maintain accurate clinical records and submit documentation according to company policy using a company‑provided device.
• Comply with all company policies and procedures to ensure high standards of care.
